We will provide you with all the support, reassurance, and professional clinical knowledge to move your loved one within 24 hours into a safe, homely, comfortable environment that offers the highest standards of care.

Having a loved one admitted to hospital is a daunting time. Families often experience panic, strain, and worry – especially if complications arise when it comes to arranging discharge from hospital. The complexities of medical issues and recovery can often impact the ability to return home immediately after a stay in hospital.
We provide a unique older person’s care service to enable discharge from hospitals into permanent or respite care. The RRT supports residents and families of those living in the Bournemouth, Christchurch, Poole, and Southampton regions, as well as those looking to receive older persons care in these areas, and to find the best care solution for them.

A team of clinical assessors

The team is made up of clinical professionals who provide the best possible advice and act as a single point of contact between you and the hospital. The team has strong working relationships with the NHS hospital discharge teams. Therefore, you can feel confident that they will liaise closely with the hospital directly, ascertain all the information you need to know about your loved one’s medical condition and situation, and relay the verified details back to you – allowing you the time to focus on their wellbeing and supporting them emotionally.

The Rapid Response Team provides you with a single point of contact and alleviates the potential frustrations of navigating the hospital system to be put through to the correct ward and seek updates from the right person. We aim to make the process of moving from hospital to care home a simple and smooth process – and take away the worry, confusion, and burden that comes with organising care for a relative. We gather all the details so that you can make an informed decision about the next step and feel reassured that your loved one is in the safest of hands.

We will arrange a healthcare assessment and a care solution within 24 hours

If you need guidance on how to get your loved one out of hospital and into permanent or temporary care, call the RRT and we will take care of everything for you – allowing you and your family the breathing room to pause and consider what is best for your relative.

The RRT will complete a full healthcare assessment of your loved one within 24 hours. Whether you require a permanent placement within a care home, or a short-term solution until your loved one can return home again, the team can offer a choice of Encore care homes to suit the resident and family’s requirements. We even can arrange same-day admissions into our care homes.

What will happen when you call RRT:

Step 1

You will speak to George or Sebastian.

Step 2

They will gather some details from you about your loved one, such as name, date of birth, address, and a care background summary, which will take 5-10 minutes. Collecting this personal information allows the team to identify your loved one with the hospital and obtain access to their records.

Step 3

They will get in touch with the hospital and gather all the information that you need to know and will act as your single point of contact from there – available to talk to you whenever you need to, whatever time of day or night.

Step 4

They will call you back to relay the most up to date information from the hospital.

Step 5

They will arrange to visit your loved one to do a full healthcare assessment, which can be organised for a time that suits you if you would also like to be present.  This can be done within 24 hours.

Step 6

If you decide that a short or long-term stay in an Encore care home is the next step, the Rapid Response Team will make all the arrangements for you and facilitate the whole process; from discharge from the hospital to admission into the care home – ensuring a smooth and stress-free process for you and your loved one. 

Step 7

Your loved one will be moved out of hospital and into a welcoming, safe, and beautiful home – which can be within 24 hours of your first call to the RRT.

Sebastian brings many skills to his role as Clinical Assessor, having successfully facilitated over 500 admissions into our care homes, supporting families and loved ones to navigate the system and get the best results possible.
