Certificate Achievement for End of Life Care

We are delighted to announce that Hamble Heights has received the certificate for delivering the 6 Steps End of Life Programme.

This is a fantastic achievement and is due to the hard work and commitment from Sue Davy (Unit Manager) and Tina Hughes (Senior HCA) who have secured the certificate status for Hamble Heights.

The Six Steps Programme

The Six Steps to Success programme is based upon a workshop format.  It addresses the core phases of End of Life Care within a cycle involving six steps which are as follows:

  • 1: discussions as End of Life approaches
  • 2: assessment, care planning and review
  • 3: co-ordination of care
  • 4: delivery of high quality care
  • 5: care in last days of life
  • 6: care after death

Hamble Heights is now certified in not only recognising end of life situations but also managing them more effectively. This means working in partnership with the individuals, their families and carers, as well as other organisations to deliver the best quality of care.

The benefits for successfully completing this programme ensures the improvement of practice within the home.  Therefore, it also improves communications with local GPs and multi-disciplinary teams.  In turn, this achievement helps to keep up to date on local and national policy and best practice.

Six Steps Programme increases the confidence amongst staff to work with and support End of Life residents and their families.  The training enhances staff satisfaction, skills and support; hence it assists staff with supporting residents around their choices for end of life care.  It provides a better understanding of any need for change and how to implement this effectively.
